Most medical case studies follow a predictable structure. Mastering this structure means you can focus your energy on the clinical thinking rather than figuring out where things go.
Patient Presentation
Start with the basics — age, sex, chief complaint, and the context of the encounter. Keep it concise. You're setting the scene, not writing a biography. One or two sentences is enough.
Example
A 34-year-old woman presents to the emergency department with three days of worsening right lower quadrant pain.
History of Present Illness
Tell the story of the illness — onset, character, location, duration, severity, timing, modifying factors, and associated symptoms. Use the patient's own language where it adds color, but keep the narrative tight. Every detail you include should be there for a reason.
The Relevance Filter
Ask yourself: does this detail change the differential or affect management? If the answer is no, cut it. A common mistake is either including everything or including too little.
Past Medical History, Medications, and Allergies
List relevant past medical history, current medications, and allergies. Relevant is the operative word. If the patient has a history of migraines and you're writing a case about appendicitis, the migraines probably don't belong here. If they're on immunosuppressants, that absolutely does.
Social and Family History
Brief and relevant. Smoking, alcohol, drug use, occupation, living situation, and family history of conditions related to the presentation. This section is frequently underwritten by students and frequently matters more than they expect — especially for presentations where lifestyle, environment, or genetics are part of the story.
Review of Systems
A targeted review of systems related to the chief complaint. You don't need to cover every system — just the ones that could inform your differential or rule something in or out.
Physical Examination Findings
Document the relevant positive and negative findings. Negatives matter here — "no rebound tenderness" and "no Murphy's sign" tell the reader something. Structure this by system: vitals, general appearance, then organ system by organ system.
Diagnostic Workup
Labs, imaging, and any other studies ordered. Include the results and — importantly — explain why each was ordered. This is where your clinical reasoning starts to show. Anyone can list a CBC and a CMP. The case study is stronger when you explain what you were looking for and what the results told you.
Differential Diagnosis
List your differential in order of likelihood, with brief reasoning for each. Include at least one must-not-miss diagnosis even if you think it's unlikely. A strong differential shows you understand the clinical picture and thought systematically rather than pattern-matching.
What Makes a Strong Differential
It's not about listing more diagnoses. It's about showing you considered a range of possibilities and used the clinical data to systematically narrow them down. Include your reasoning — why each is likely or unlikely.
Final Diagnosis
State the diagnosis and explain how you arrived at it. Walk through which diagnoses you ruled in, which you ruled out, and why. This is the payoff of the case — it should feel like the logical conclusion of everything that came before it.
Management and Treatment
What was done and why. Include immediate management, definitive treatment, follow-up considerations, and any patient education that was relevant. For complex cases you might also address what was considered and rejected.
Discussion and Clinical Pearls
This is what separates a case study from a medical record. Zoom out — what does this case teach? What's the broader lesson? Is there something about the presentation that was atypical? Something about the workup or management worth highlighting for other learners? This section is where your voice as a clinician comes through most clearly.
Common Mistakes Medical Students Make in Case Studies
Writing a timeline instead of a narrative
A list of events in chronological order is not a case study. The reader needs to understand not just what happened but why each decision was made. Write it like you're explaining the case to a colleague, not filing a chart note.
Burying the lead
Your chief complaint and most relevant history should come first. Don't make the reader wade through three paragraphs of social history before they understand why the patient presented.
Weak differentials
Listing three diagnoses that all present identically and picking the right one without real reasoning isn't a differential — it's a guess with extra steps. A strong differential shows you considered a range of possibilities and used the clinical data to systematically narrow them down.
Ignoring pertinent negatives
What the patient doesn't have is often as important as what they do. A case about chest pain without documenting the absence of diaphoresis, radiation, and associated dyspnea is incomplete.
Skipping the discussion
Students often run out of steam by the time they get to the discussion and write two sentences. This section is what makes the case worth reading. Give it the attention it deserves.
